What documentation do I have to submit to collect my life insurance benefit if I suffer any of the contingencies covered by the policy?
Life insurance offers complementary coverage to death from any cause. These coverages can be disability due to illness, disability due to accident and serious illness. If you have contracted these coverages and suffer any of these contingencies, it will be necessary to present a series of documents that prove the situation in order to collect the benefit.
In case of disability due to illness, the documentation that must be presented to collect the life insurance benefit is the following:
- Resolution of the Social Security on the disability granted with date of economic effects
- Report of the Social Security Disability Assessment Medical Unit (UMM)
- Medical report with the cause of disability of the insured with the date of diagnosis of the disease caused
- Copy of the medical record issued by your Primary Care doctor, with the medical history and date of diagnosis, including a summary of all the insured’s conditions, specifying the description and date of diagnosis of each suffering
- History of sick leave caused by them (must be requested at the Ministry of Equality, Health and Social Policies)
- Photocopy of the insured’s valid ID
- Payment account number of the insured benefit
In case of disability due to accident, it will be necessary to present the following documents:
- Resolution of the Social Security on the disability granted with date of economic effects
- Report of the Social Security Disability Assessment Medical Unit (UMM)
- Judicial and/or police proceedings with autopsy and toxicology report
- History of sick leave caused by them (must be requested at the Ministry of Equality, Health and Social Policies)
- Photocopy of the insured’s valid ID
- Payment account number of the insured benefit
In case of serious illness, the following documentation must be presented:
- Medical report with the date of diagnosis of the serious illness, severity, stage in which it is located and results of the pathological abnormality, if it has been carried out
- Photocopy of the insured’s valid ID
- Payment account number of the insured benefit
